At ASPIRE disABILITY, our Specialist Support Coordinators can:

Help with Big Challenges and Crisis Situations:

• Give extra support for people with many needs.
• Help in emergencies, fix problems with services, and manage high-risk issues.
• Create and follow plans for emergencies to make sure care goes smoothly.
• Keep services running well during a crisis or unexpected event.

Solve Problems and Manage Complex Cases:

• Help identify and work on problems that might prevent you from getting the support you need.
• Handle cases that need expert knowledge, especially for high-level support needs.
• Identify and understand barriers that affect your access to supports.
• Find and use solutions to overcome these barriers.

Support and Coordinate Your Services:

• Connect with many service providers and ensure all services work together.
• Create a service plan that helps you reach your goals.
• Set up ways for your support team to talk and work together.
• Help you, your family, and carers solve problems and unexpected situations.

Build Skills and Confidence:

• Find a wide range of supports to help you use your plan well.
• Help you build skills and confidence for greater independence.
